The mediating role of health behaviors in the association between depression, anxiety and cancer incidence: an individual participant data meta-analysis

Background
Although behavioral mechanisms in the association among depression, anxiety, and cancer are plausible, few studies have empirically studied mediation by health behaviors. We aimed to examine the mediating role of several health behaviors in the associations among depression, anxiety, and the incidence of various cancer types (overall, breast, prostate, lung, colorectal, smoking-related, and alcohol-related cancers).
MethodsTwo-stage individual participant data meta-analyses were performed based on 18 cohorts within the Psychosocial Factors and Cancer Incidence consortium that had a measure of depression or anxiety (N = 319 613, cancer incidence = 25 803). Health behaviors included smoking, physical inactivity, alcohol use, body mass index (BMI), sedentary behavior, and sleep duration and quality. In stage one, path-specific regression estimates were obtained in each cohort. In stage two, cohort-specific estimates were pooled using random-effects multivariate meta-analysis, and natural indirect effects (i.e. mediating effects) were calculated as hazard ratios (HRs).
ResultsSmoking (HRs range 1.04–1.10) and physical inactivity (HRs range 1.01–1.02) significantly mediated the associations among depression, anxiety, and lung cancer. Smoking was also a mediator for smoking-related cancers (HRs range 1.03–1.06). There was mediation by health behaviors, especially smoking, physical inactivity, alcohol use, and a higher BMI, in the associations among depression, anxiety, and overall cancer or other types of cancer, but effects were small (HRs generally below 1.01).
ConclusionsSmoking constitutes a mediating pathway linking depression and anxiety to lung cancer and smoking-related cancers. Our findings underline the importance of smoking cessation interventions for persons with depression or anxiety.

Background
Early studies of common mental disorders (CMDs) during the COVID-19 pandemic mainly report increases; however, more recent findings have been mixed. Also, studies assessing the effects of restriction measures on CMDs show varied results. The aim of this meta-analysis was to assess changes in levels of CMDs from pre-/early to during the pandemic and the effects of restriction policies in the European population.
MethodsWe searched for studies assessing both pre-pandemic and peri-pandemic self-reported emotional distress and symptoms of depression or anxiety among nationally/regionally representative samples in Europe and collected microdata from those studies. Estimates of corona containment index were related to changes in CMDs using random-effects meta-regression.
ResultsOur search strategy resulted in findings from 15 datasets drawn from 8 European countries being included in the meta-analysis. There was no evidence of change in the prevalence of emotional distress, anxiety, or depression from before to during the pandemic; but from early pandemic periods to later periods, there were significant decreases in emotional distress and anxiety. Increased school restrictions and social distancing were associated with small increases in self-reported emotional distress.
ConclusionsDespite initial concerns of increased emotional distress and mental illness due to the COVID-19 pandemic, the results from this meta-analysis indicate that there was a decrease in emotional distress and no change in anxiety or depression in the general population in Europe. Overall, our findings support the importance of strong governance when implementing periodic and robust restriction measures to combat the spread of COVID-19.

Summary
Introduction
As in other medical specialties the increasing emphasis on medical audit should be welcomed by the paediatricians. The need to review critically specific aspects of paediatric care is of particular importance. What is implemented by paediatricians may have long-term consequences, potentially into adult life. These consequences may on occasions not be appreciated if they subsequently present only to the adult physician or if adequate follow up is not achieved. It may be difficult to measure every aspect of paediatrics and child health, although a wide spectrum of paediatric care can be covered from child health surveillance, neonatology, paediatric surgery and general paediatrics. It should begin with perinatal care and the developing foetus and thus by definition cross into obstetrics. Although audit in the form of, for example, perinatal meetings have taken place in the past, the recent impetus in initiating more formal audit should remind us that previously some new procedures, drugs and techniques have been accepted with enthusiasm without critical review. Intrapartum foetal monitoring of the full-term baby is so accepted by some that it is thought negligent not to monitor during labour, although a recent study now questions this. Before accepting specific audit procedures as a measure of quality of care then we have to be certain that our guidelines are reliable, as shown in the preceding example.
